• If you as a current client or applicant would like an advocate who helps you (i.e., social worker, case manager, relative) to receive copies of DCHA correspondence sent to you , fill out this form with their information and return it to us. If you are a current client, you should return it to your caseworker (or, you can send it to info@dcha.net if you are not sure who your caseworker is or you do not have their email). If you are an applicant on our waiting list(s), you should send it to eligibility@dcha.net.
